Practice Based Commissioning

Since March 2007 Aston PBC Group have been commissioned by Knowsley PCT to provide a ‘Diabetes Intermediate Service’ to its patient list, which currently stands at approximately 28,000 patients and covers 7 sites.Practice Based Commissioning

Automated referrals

Lynne Nightingale explains how Vision has helped them to provide this service, “We have utilised Vision to support the project from the beginning, and have developed pathways and protocols to maintain the flow of clinical and non-clinical information. Referrals are generated electronically through Vision and Read codes used to record information on activity levels and consultations.”

Vision makes life easier

The system also helps Lynne with the administrative aspects of the service, “Vision makes the job easier and more efficient when collating data for contractual commitments on a monthly and quarterly basis. It’s quick and easy to generate a range of key performance information including patient demographics, diagnostic testing and trend analysis of clinical indicators such as HbA1, cholesterol and blood pressure. Related interventions are recorded and we use Vision’s Search and Report facilities to collate data and numbers.”

Lynne has been pleased with the help and support she has received from INPS, “The Vision helpline has offered practical solutions when needed, and have given us advice on the best way for us to apply Vision to our service.”
