Vision Community Care

Vision Community Care helps District Nurses, Health Visitors and AHPs by making it easier to find their patients within Vision and record information quickly. It’s great for practices too because it encourages data to be recorded consistently, leading to improved data quality.

Vision Community Care allows data from community-based healthcare professionals to be stored within Vision, giving the Primary Care Team vital information about their patients that would not normally be available.

The package includes training and consultancy that will allow you to get the most from Vision’s Community Caseload facility and the new Vision Guidelines (templates for quick and consistent data entry) that have been written specifically for District Nurses and Health Visitors.

• The practice gathers consistent, high quality data from all Vision users
• ‘Community Caseload’ facility in Vision restricts the AHP to access only his/her own caseload of patients and does not allow them to see other patients within the practice dataset. Caseloads can be managed and monitored by the nurse Team Leader, Nurse or Manager.

Pre-defined community guidelines and audits will be supplied along with the community caseload activation and training.

Please note: Modifications to the data entry templates and audits are not included in the cost and will be priced depending on requirements.
