Cancer Referral Proformas

This zip file contains documents adapted from the Department of Health's Cancer Referral Proformas to enable completion of the referral template within Vision. The referral can then be faxed/emailed to the Urgent Referral Team.

All documents are in Word 7 (Office 97) format and contain Vision merge marks, check boxes and drop down form fields. You must be using Word, not High Edit, as the wordprocessing system in Vision.

After unzipping, please print and read the two documents "Cancer Referral Proformas - Instructions" and "Open Data Source Error When Opening Word Templates in Vision".
