Choose and Book Referral Templates

These officially designed templates can be used for all diagnostic referrals to the following relevant services made through Choose and Book: audiology, cardiac, endoscopy, gastrointestinal, imaging, neurophysiology, respiratory, urodynamics, vascular. 

Place the templates in P:\WORDPRPOC\TEMPLATES and use during Referral Add.  Practices should first make sure that the option Protect Froms on Creation is ticked (Utilities - Word Processor - Options - Merge field options) to allow easy tabbing between entries on the forms.
